The membership is $1,495 per year, and it's eligible for payment through a Health Spending Account (HSA).

Everything in the membership is included: your dedicated Nurse Practitioner and care team, unlimited in-person and virtual visits, your Annual Longevity Assessment with advanced biomarkers, quarterly InBody scans, and your personalized 12-month longevity plan.

A small number of services fall outside the membership: some minor procedures, completion of medical forms, and missed-appointment fees. We'll always tell you about any additional cost before it's charged.

Nurse Practitioners are not permitted to bill OHIP for their services, so we cannot accept OHIP as payment for the membership.

However, OHIP still covers medically necessary care that your Nurse Practitioner orders. Lab tests, diagnostic imaging, and specialist visits that are medically necessary are OHIP-covered, so you won't pay out of pocket for them. Advanced longevity biomarkers that OHIP doesn't cover are included in your annual membership.

Yes. Because our Nurse Practitioners don't bill OHIP, joining Optimal doesn't affect your family doctor financially, and there's no "outside use" penalty for either of you.

Many members keep their family doctor and use Optimal as a second, on-demand option when they need it, whether that's for a cough or cold, a specific treatment, or a longevity-focused plan their current provider doesn't offer. It's also a better alternative to busy walk-in clinics with long wait times, or the limited care available through virtual-only telemedicine.

With sufficient notice, there is no fee.

Members who cancel their membership will not be charged a fee if they cancel more than 60 days before the expiration of their annual membership ($0).

Late cancellations still require us to plan a transition of care to your new provider. Members who provide 30-60 days notice to cancel are charged a late cancellation fee of $145. If less than 30 days notice is provided, the fee is $290.

Your care is led by a Nurse Practitioner. Nurse Practitioners are registered with the College of Nurses of Ontario. They are not naturopaths.

Primary Healthcare Nurse Practitioners (PHC-NPs) can diagnose and treat illness, prescribe medications, manage chronic conditions like diabetes or high blood pressure, complete annual health exams, order lab tests and imaging, and refer you to specialists.

The main difference is the training route. Family physicians go to medical school and then complete additional training in primary care. Nurse Practitioners go to nursing school and then complete additional training in primary care.

For time-sensitive concerns, you can book a same or next-day appointment. For routine care that is not urgent but still requires a prompt visit, you'll see your dedicated provider within one week.

If you need a specialist, such as a cardiologist, ENT, or neurologist, your Nurse Practitioner can refer you. We make the referral, track the appointment, and follow up to confirm. Specialist visits are OHIP-covered.
